ACHA may request information related to your heart defect or congenital heart disease related health issues, which may be gathered in ways such as our patient/family membership form, Peer Mentor request form, and in one-on-one conversations with ACHA staff.
It is our promise that this information will never be shared outside of ACHA staff (and, only when necessary, our Board of Directors, Medical Advisory Board, or Heart to Heart Peer Mentors) without your consent. However, we will use this information to communicate with you about ACHA’s programs, services and activities in order to best serve you.
