Please prepare the following items:

（1）   Certificate of Tax Payment for Light Motor Vehicle Tax (Vehicle Inspection Test Use) Certificate Request Form (For applications via mail)
（Can be downloaded below）

（2）Copy of automobile inspection certificate (Shaken-sho)

（3）Copy of a document confirming the applicant’s identity（Must include name, address, and birth date）

Examples: Residence card, passport, driver’s license, etc. (Please take a copy of the side displaying the applicant’s name and address)

（4）Return envelope (Please attach a stamp worth 84 yen and write the applicant’s name and address)

（5）Original copy of your receipt for Light Motor Vehicle Tax Payment（If the payment was made recently）

必要ひつようなもの ：

（１）軽自動車けいじどうしゃ税ぜい納税のうぜい証明しょうめい（継続けいぞく検査けんさ用よう）交付こうふ申請しんせい書しょ（郵送ゆうそう申請しんせい用よう）

（下記かきよりダウンロードできます。）

（２）車検しゃけん証しょうの写うつし

（３）申請しんせい者しゃの本人ほんにん確認かくにん資料しりょう（名前なまえ、住所じゅうしょ、生年月日せいねんがっぴの記載きさいがあるもの）の写うつし

例れい：在留ざいりゅうカード、パスポート、運転うんてん免許めんきょ証しょうなど（名前なまえと、今いまの住所じゅうしょが書かいてある面めんをコピーしてください。）

（４）返信へんしん用よう封筒ふうとう（切手きって（８４円えん）を貼はって、申請しんせい者しゃの住所じゅうしょ、名前なまえを書かいてください。）

（５）軽自動車けいじどうしゃ税ぜい領収りょうしゅう済ずみ証しょうの原本げんぽん（申請しんせいの直近ちょっきんで納付のうふされた場合ばあい）

※手数料てすうりょうは無料むりょうです。

 ※※本人ほんにん以外いがいが申請しんせいする場合ばあいは、必要ひつようなものが異ことなりますのでお問とい合あわせください。
