    Alcohol use disorder is a serious health problem in college students. Alcohol Use Disorders Identification Test (AUDIT) is a 10-item screening tool to assess alcohol consumption, drinking behaviors, and alcohol-related problems. AUDIT-C is a short version of AUDIT, consisting of consumption items 1-3. However, the optimal cutoff values of AUDIT and AUDIT-C for detecting excessive drinking are not available for Japanese college students. The aim of this study was to evaluate the validity of cutoff points of AUDIT and AUDIT-C for detecting moderate drinking, heavy drinking and binge drinking among Japanese college students. The cross-sectional study was based on an anonymous, self-administered questionnaire. In January 2017, we sampled college students and graduate students aged 20 years or older during annual health examinations at Mie University in Japan. Two thousand students underwent health examinations, and the eligible subjects were 1,600, including 152 (9.5%) moderate drinkers, 58 (3.6%) heavy drinkers and 666 (41.6%) binge drinkers. ROC (receiver operating characteristic) curve analysis showed that the optimal cutoff values for moderate drinking, heavy drinking and binge drinking were 5, 8 and 5 for men and 4, 7 and 4 for women in AUDIT; and 4, 7 and 4 for men and 4, 7 and 4 for women in AUDIT-C, respectively. Moderate drinking is considered unsafe drinking. Therefore, the optimal cutoff values for moderate drinking (5 for men and 4 for women in AUDIT and 4 for both sexes in AUDIT-C) are important parameters for prevention of alcohol use disorder in Japanese college students.

    Objective This study retrospectively explores the objective signs of imminent suicide risk in psychiatric in-patients.

    Design The study analysed the diagnostic and nursing records of a psychiatric hospital that covered the last 14 days before the suicide attempts of 18 people, who, between March 2008 and July 2019, were found to have died by suicide during their hospital stay.

    Methods Three professionals used a fishbone diagram to separately identify the factors that led to each person’s suicide, the objective signs that indicated imminent suicide risk, possible preventive strategies, and other observations. They compared their findings and used the KJ method (Kawakita Jiro Method) to categorise the items on which they all agreed.

    Results Objective signs of imminent suicide risk were condensed into five categories: ‘signs emanating from the patient’, ‘signs gleaned through engagement’, ‘signs from response to treatment’, ‘signs associated with reports from the family’, and ‘signs inferred from multiple sources of information’. Five categories describing issues with the way in which the hospital staff handled information were extracted, namely ‘omission in diagnostic records during admission’, ‘omission in conference records’, ‘communication lapse during transfer’, ‘need for integrated information’, and ‘systemic issues’.

    Conclusions The findings offer insights on assessing suicide risk and preventing suicide.

    Japan has a high suicide mortality rate compared to other developed countries. To reduce suicide mortality in Japan, it is important to systematically analyze factors related to death of patients with suicide attempt. This study aimed to analyze the characteristics of patients with suicide attempt, and the factors related to their death using the Diagnosis Procedure Combination (DPC) data— a nationally representative inpatient database. We collected 81,407 cases of suicide attempt from 2016 to 2018 from DPC data and performed a multilevel logistic analysis of factors associated with death discharges. The analysis results showed that patients who received psychiatric liaison care had a lower mortality rate, but only 0.6% of surviving patients received psychiatric liaison care after admission. The odds ratio (OR) of death was high for hanging (28.86; p < 0.001) and jumping (16.28; p < 0.001), compared to wrist cutting. Patients without a psychiatric diagnosis were more likely to choose means such as hanging (14.1%) than those with a psychiatric disorder. The weekend cases had a higher OR of death than weekday (Wednesday as reference) cases (Friday 1.14, p = 0.011; Saturday 1.60, p < 0.001; Sunday 1.67, p < 0.001). Based on these findings, we suggest that improving the availability and quality of psychiatric care in acute care hospitals and primary care settings, as well as enhancing the emergency department system on weekends, could help reduce the mortality of suicide attempts.

    Diesel exhaust (DE) is one of the major air pollutants in the world. DE disrupts steroid hormone levels, which may result from the disruption of spermatogenesis. Steroidogenesis occurs not only in the testis but also in the brain. Therefore, we investigated the effects of nanoparticle-rich DE (NR-DE) on steroidogenesis in both the testis and hippocampus. Exposure to NR-DE at concentrations comparable to the environmental standard for particulate matters 2.5 (PM2.5) in Japan increased plasma testosterone level. This exposure increased the expression levels of genes involved in steroidogenesis in the testis, but not in the hippocampus, suggesting that NR-DE disrupts steroid hormone balance. This finding suggests the need to reconsider the environmental limit of PM2.5 in Japan.

    Granulation tissue which is responsible for root resorption of deciduous tooth lies between root of the deciduous tooth and its permanent tooth germ. This tissue is called “root resorbing tissue”. Its bone-resorbing activity was investigated in vitro. Bovine root-resorbing tissue was cultured in close contact with 45Ca-labeled dead calvaria of rats. Bone-resorbing activity was determined by measuring 45Ca released from labeled calvaria during culture. It was found that only the root-resorbing tissue which was rich in odontoclasts and had a good blood supply in its surface layer had bone-resorbing ability, and that bone resorption occurred only when it was placed in close contact with calvarium. The root resorbing tissue which was poor in odontoclasts and blood vessels failed to stimulate bone resorption. Bone resorption by the root-resorbing tissue was enhanced markedly by 25-hydroxy-vitamin D3 or heparin, but not by larger amounts of parathyroid hormone, vitamin D3, and dihydrotachysterol when added to the culture.

    Several reports have suggested that semen quality in normal men is declining. Given the lack of consensus about the effects of diesel exhaust (DE) on the male reproductive system, we conducted various experiments. We examined the effect of the exposure of mature male mice to DE for 6 months on the male reproductive system. Daily sperm production per gram from the testes dose-dependently decreased with exposure to DE. Next, we investigated the effect of exposure of pregnant mice to DE on male gonad development at the level of mRNA expression. Expression of mRNAs for steroidogenic factor (Ad4BP/SF-1) and Müllerian inhibiting substance (MIS) decreased significantly in male fetuses when maternal mice were exposed to DE for 8 hr per day between days 2 and 13 post coitum. In addition, DE exposure during the fetal period may have some influence on the male reproductive function in newborn mice. In utero DE exposure caused testosterone levels to increase in newborn male mice at age 4 weeks. These findings indicate that exposure to DE may influence the male reproductive system. Further studies are necessary to elucidate the mechanism of the effects of DE on the male reproductive system and to define which classes of compounds are responsible for the changes in the male reproductive system.

    Xylooligosaccharides (XOS) and arabinoxylooligosaccharides (AXOS), potential prebiotics, can be produced from rice bran arabinoxylan (RBAX) using commercial endoxylanase enzymes. However, differences in rice bran cultivars and endoxylanase enzyme types may affect extracted oligosaccharides (OS) yields. This study investigated extracted OS structures derived from three different RBAX using two commercial endoxylanase enzymes. RBAX extracted from commercially defatted rice bran (CDRB) yielded the greatest OS amount (456.69 mg/g RBAX), followed by that of the San-Pah-Tawng1 (SPT1) cultivar (231.7 mg/g RBAX), and lastly, the Chai-Nat1 (CN1) cultivar (172.57 mg/g RBAX), as evidenced via Ultraflo L enzyme hydrolysis. Ultraflo Max enzyme hydrolysis produced a similar trend, however, OS amounts from all RBAX sources were remarkably lower (83.39 mg/g RBAX extracted from CDRB, 27.05 mg/g RBAX from SPT1 cultivar, and 21.53 mg/g RBAX from CN1 cultivar). Interestingly, 32-αあるふぁ-L-Araf-(1-4)-βべーた-D-xylobiose (A3X) was the primary AXOS product in all RBAX hydrolysates prepared by Ultraflo Max. RBAX extracted from CDRB solubilized by Ultraflo L (45.67% weight of RBAX) had higher OS yields than that obtained via Ultraflo Max (8.3% weight of RBAX). Ultraflo L was therefore a suitable commercial enzyme for short-chain OS conversion from RBAX.

    乳腺にゅうせん分泌ぶんぴつがん別名べつめい若年じゃくねんせいがんともよばれ,乳癌にゅうがん取扱とりあつか規約きやくにもしるされているが,まだその報告ほうこく非常ひじょうにまれである.今回こんかい筆者ひっしゃらは,穿刺せんし吸引きゅういん細胞さいぼうにて特徴とくちょうてき所見しょけんていした成人せいじん乳腺にゅうせん分泌ぶんぴつがん経験けいけんしたので報告ほうこくする.
    症例しょうれいは55さい女性じょせいで,みぎ乳腺にゅうせんしゅこぶおも訴に来院らいいんした.触診しょくしんではみぎ乳房ちぶさBED領域りょういきに1.5cmだい弾性だんせいかた,境界きょうかい明瞭めいりょう可動かどう良好りょうこうしゅこぶさわしたが,皮膚ひふ変化へんかみとめず,腋窩えきかリンパぶしさわしなかった.ちょう音波おんぱ検査けんさでは,しゅこぶさわ一致いっちして境界きょうかいするど,不整ふせいがた内部ないぶエコーはほぼ均一きんいつていエコーぞうみとめた.マンモグラフィーでは,触診しょくしんおな部位ぶいに,みち1.7cmのあたりえんやや不明瞭ふめいりょう円形えんけいしゅこぶ陰影いんえいみとめたが,微細びさい石灰せっかいぞうあきらかではなかった.以上いじょう所見しょけんより乳癌にゅうがんうたがい,穿刺せんしほそ胞診をおこなった結果けっか,Class Vでみぎ乳癌にゅうがん(T1N0 M0)と診断しんだんし,定型ていけい乳房ちぶさ切除せつじょじゅつ施行しこうした.
    細胞さいぼう所見しょけんでは,かく小体こてい目立めだかくゆうする細胞さいぼうじゅうせきないしシートじょう細胞さいぼうしゅうかたまりがみられた.細胞さいぼうしつ豊富ほうふあわそら胞や粘液ねんえきゆうしており,かく三日月みかづきがた偏在へんざいしているものもみとめられた.これらの細胞さいぼうしゅうかたまりはあたかも“ぶどうのぼうさまであった.
    組織そしき所見しょけんでは,大小だいしょう不同ふどうかん腔を形成けいせいし,かん腔内と細胞さいぼうしつにPAS,Alcian-blue陽性ようせい分泌ぶんぴつぶつみとめた.また,腫瘍しゅようあいだしつむscirrhosなぞうていし,一部いちぶ乳腺にゅうせんがい脂肪しぼう組織そしきへの浸潤しんじゅんみとめた.以上いじょう所見しょけんより乳腺にゅうせん分泌ぶんぴつがん診断しんだんした.
    なお,腫瘍しゅよう組織そしきないER,PgRはともに陰性いんせいであった.術後じゅつご経過けいか順調じゅんちょうで,現在げんざい再発さいはつ兆候ちょうこうはない.
    分泌ぶんぴつがんは.1966ねんにMcDivitらが,腫瘍しゅよう細胞さいぼう著名ちょめい分泌ぶんぴつぞうしめし,PAS陽性ようせい物質ぶっしつ細胞さいぼうない,せんぼうさま腔内にみられる組織そしきがくてき特徴とくちょうゆうする3さいから15さいの7れい若年じゃくねんせい乳癌にゅうがん最初さいしょ報告ほうこくした.しかし,1980ねんにTavassoliらは,成人せいじんにおいても同様どうよう組織そしきぞうていする乳癌にゅうがん提示ていじし,若年じゃくねんしゃのみにみられるものではないことを報告ほうこくした.筆者ひっしゃらが調しらかぎりでは,本邦ほんぽうでは14れい報告ほうこくがあるが,穿刺せんし吸引きゅういん細胞さいぼうについてべた報告ほうこくはほとんどみあたらない.Shinagawaらは最近さいきん,分泌ぶんぴつがん穿刺せんし吸引きゅういん細胞さいぼうにおいて,ほん症例しょうれいにおいてもみとめられた“ぶどうのぼうさま細胞さいぼう所見しょけん特徴とくちょうてきであることをべている.
    ほん疾患しっかんかんしては,乳癌にゅうがんより良好りょうこうという報告ほうこくもあるが,本邦ほんぽう報告ほうこく14れいについてみてみると,42%にリンパぶし転移てんいみとめられており,今後こんご症例しょうれいかさねて検討けんとうしていく必要ひつようせいがあるとおもわれる.

    The crystal and molecular structure of 4-hydroxy-2-oxo-1-pyrrolidineacetamide was determined by X-ray analysis. The molecular conformation found in the solid was compared with results of theoretical quantum mechanical calculations carried out by using an ab-initio method at the STO-3G level. Crystals (orthorhombic) of the studied nootropic agent were as follows : a=7.162 (6), b=8.852 (3), c=11.340 (8) Å ; Pbc21. The five-membered ring deviates from planarity, while the planes of the amide group and of the four atoms N (1)-C (1)-C (2)-C (4) are almost perpendicular in cis arrangement, with the 4-hydroxy group in the axial position. In the crystals, molecules are linked by two intermolecular hydrogen-bonds, namely O (3)…O (2) and N (2)…O (1), which are 2.77 and 2.92 Å long, respectively. Results of theoretical calculations indicate for the free molecule a twisted cis conformation, with the 4-hydroxy goup in the axial position, as the most preferred one. This appears to be determined by the intramolecular hydrogen-bond between the O (1) atom and amide group hydrogen atoms. The relative conformational energies indicate that several conformations about the minimum can be populated in the solution state. The geometry and conformational profile of the pharmacologically active investigated molecule were found to reproduce, with the exception of the puckered conformation of the fivemembered ring, those previously determined for the related analogous nootropic agent 2-oxo-1-pyrrolidineacetamide (piracetam), whose chemical structure differs only in the lack of the 4-hydroxy group in the β-position to the carbonyl group.

    Methyl esters of acetylene carboxylic acids (2 and 3), an α-bromo acid (4), and α, α-difluoro acids (5 and 6) were synthesized as analogues of the C-24 carboxylic acid which is an intermediate in the microbial degradation of sterol side chains, and these compounds were shown to be inhibitors of the degradation reaction.

    The purpose of this study was to compare the effects of zinc oxide-eugenol (ZOE) and calcium hydroxide/iodoform paste (Vitapex), as root canal filling materials in pulpectomy, on delaying the root resorption of primary molars without permanent successors. Animal models without permanent successors were surgically established in beagle dogs. Root resorption was observed via periapical radiographs. The onset of root resorption of primary mandibular molars without successors occurred later (p<0.05) than physiologic resorption. ZOE pulpectomy clearly delayed the root resorption of primary molars without permanent successors (p<0.05), whereas resorption of primary molars with Vitapex pulpectomy started at almost the same time as physiologic resorption. Compared with Vitapex, ZOE was a more effective root canal filling material in delaying the root resorption of primary molars.

    ほん研究けんきゅうは,障害しょうがい幼児ようじ問題もんだい行動こうどう母親ははおやのストレス認知にんち(育児いくじ負担ふたんかん)の関連かんれんせいについてあきらかにすることを目的もくてきとした。調査ちょうさ参加さんかしゃは,S県内けんない障害しょうがい通園つうえん施設しせつ12施設しせつ通園つうえんする母親ははおや319にん養護ようご学校がっこう11こう在籍ざいせきする母親ははおや1,030にんであった。調査ちょうさ内容ないよう属性ぞくせい(せい,年齢ねんれい,学年がくねん,障害しょうがい,医学いがくてき分類ぶんるいめい,問題もんだい行動こうどう),母親ははおや属性ぞくせい(年齢ねんれい,かず,家族かぞく構成こうせい,就労しゅうろう),育児いくじ負担ふたんかんとした。問題もんだい行動こうどう母親ははおや育児いくじまけかん関係かんけいは,潜在せんざい変数へんすうもちいたじゅう回帰かいきモデル(多重たじゅう指標しひょうモデル)を仮定かていし,構造こうぞう方程式ほうていしきモデリングをもちいて,その因果いんがモデルの適合てきごうせい変数へんすうあいだ関係かんけい程度ていど検討けんとうした。問題もんだい行動こうどう母親ははおや育児いくじまけかんおよぼす影響えいきょう程度ていどは,寄与きよりつで31.0 %であった。とくに,結果けっか問題もんだい行動こうどうの3つの因子いんしのうちの「感情かんじょう統制とうせい困難こんなん」の因子いんしが,育児いくじ負担ふたんかんの4つの因子いんしのうちの「たいする拒否きょひ感情かんじょう」と「育児いくじたいする否定ひてい感情かんじょう」のふたつの因子いんしにより関係かんけいしていることが示唆しさされた。以上いじょう結果けっか基礎きそに,問題もんだい行動こうどうにどのように専門せんもん対処たいしょすべきかについて討論とうろんした。

    The aim of the study is to evaluate if the occupational exposure to urban pollutants could cause alterations on 17-alfa-hydroxy-progesterone plasma levels and related diseases in male traffic policemen. 17-α-OH-P is synthesized in Leydig cells and in adrenals; it influences spermiogenesis, acrosoma reaction, testosterone biosynthesis, blocking of gonadotropin secretion; it regulates learning, memory and sleep. After excluding principal confounding factors, i.e., rotating or night shifts, exposure to solvents, paints and pesticides during time-off and smoking, traffic policemen were matched with controls by age, working life and drinking habit. Finally, 112 traffic policemen and 112 controls were included in the study. In traffic policemen 17-α-OH-P mean values were significantly higher vs. controls. The distribution of 17-α-OH-P values in both groups was significant. An increased frequency of fertility disorders referred to the questionnaire items were found in traffic policemen vs. controls, but the difference was not significant. The occupational exposure to low doses of chemical urban stressor, interacting with and adding to the psychosocial ones, could alter plasma 17-α-OH-P concentrations in traffic policemen vs. controls. 17-α-OH-P could be used as an early biological marker, even before the onset of the reproductive and mental health diseases.

    A stereoselective synthesis of demethylgorgosterol (2) is described. Alkylation at the C-22 position of the steroidal 23-aldehyde (5) was achieved by Claisen rearrangement of the piperidine enamine to give the (22S)-22-aldehyde (10a) predominantly. Compound 10a was transformed to the 22-hydroxymethyl-24-aldehyde mesylate (15a). When the mesylate was treated with potassium t-butoxide, the 22, 23-cyclopropyl 24-aldehyde (19a) was obtained in high yield. An isopropyl group was introduced at the C-24 position by means of the Grignard reaction and subsequently the hydroxy group was oxidized to provide the 24-ketone (25a). Wittig reaction of 25a followed by hydroboration and then LiAIH4 reduction of the mesylate gave 2, which was identical with the natural compound in all physical properties. Three other epimers, the (22R, 23R, 24S)-isomer (32), (22S, 23S, 24R)-isomer (34) and (22S, 23S, 24S)-isomer (35), were prepared by the same procedure. These four isomers can be separated by gas-liquid chromatography on a glass capillary column coated with OV-17.

    河原かわはら生態せいたいリスク評価ひょうかりくいきおよび水域すいいき生物せいぶつ多様たようせい保全ほぜんするために必要ひつようである.河原かわらせい生態せいたいリスク評価ひょうかには,られる知見ちけん生態せいたいがくてき関連かんれんづけるうえで,河原かわはら棲息せいそくするたねもちいることが重要じゅうようである.ほん研究けんきゅう目的もくてきは,どうをモデル化学かがく物質ぶっしつとして,日本にっぽん河原かわらられるカザリゲトビムシぞく一種いっしゅIsotomurus cf. qinpuensis)をもちいた回避かいひ試験しけん実行じっこう可能かのうせい調しらべることである.感受性かんじゅせい試験しけん成果せいか比較ひかくするために,標準ひょうじゅん回避かいひ試験しけんもちいられているオオフォルソムトビムシ(Folsomia candida)をもちいた回避かいひ試験しけんおこなった.その結果けっか回避かいひ試験しけんりょうたねとも妥当だとうなものとしておこなうことができた.りょうたねにおいて,どう濃度のうどたかくなるにともない回避かいひりつたかくなった.回避かいひりつの10% および50% 影響えいきょう濃度のうど (95% 信頼しんらい区間くかん) はそれぞれ,カザリゲトビムシぞく一種いっしゅにおいては0.8 (0.0–4.4)およ2821.0 (0.0–10,009.0)mg Cu/kg, オオフォルソムトビムシにおいては 0.7 (0.0–1.7)および101.9 (42.1–161.8)mg Cu/kg であった.これらの結果けっかから,カザリゲトビムシぞく一種いっしゅもちいた回避かいひ試験しけん実行じっこう可能かのうであることがしめされた.河原かわはら生態せいたいがくてき関連かんれんせいたか生態せいたいリスク評価ひょうかおこなうためには,ほんたねもちいた回避かいひ試験しけん利用りようされることが期待きたいされる.ほんしゅ飼育しいくけい確立かくりつ生態せいたい解明かいめいすることで,ほんたねもちいた試験しけん利便りべんせいたかまるとかんがえられた.

    森林しんりんないにおける大気たいきちゅうガス・粒子りゅうししょうたまき芳香ほうこうぞく炭化たんか水素すいそ(PAHs)の同時どうじ観測かんそくてきした試料しりょう採取さいしゅほうおよびその迅速じんそくぜん処理しょりほう開発かいはつおこなった.あつまり装置そうちは,粒子りゅうしあつまりようサンプラー,新規しんき開発かいはつしたガスじょうPAHsあつまりようカートリッジ〔内径ないけい35 mm,スチレンジビニルベンゼンども重合じゅうごうたい樹脂じゅし以下いかXAD-2)5.0 g〕およびローボリウムポンプからなり,吸引きゅういん流量りゅうりょうは20 L/minとした.XAD-2からのPAHsの抽出ちゅうしゅつは,ジクロロメタンによるちょう音波おんぱ抽出ちゅうしゅつとし,そのじょつぶ濃縮のうしゅくおよ分析ぶんせきおこなった.対象たいしょうとする8しゅのPAHsのぜん処理しょり操作そうさたいする回収かいしゅうりつは80~100% であり,また,20分間ふんかん2かいちょう音波おんぱ抽出ちゅうしゅつにより95% 以上いじょう抽出ちゅうしゅつ効率こうりつることができた.さらに,開発かいはつしたカートリッジによる大気たいきちゅうガスじょうPAHsのあつまり効率こうりつは,夏季かき冬季とうきともに95% 以上いじょうであった.本法ほんぽうもちいたFM多摩丘陵たまきゅうりょう東京とうきょう八王子はちおうじ内高うちだかやく12 mのタワー上部じょうぶ下部かぶでの観測かんそく結果けっかから,森林しんりんによる大気たいきちゅうPAHsの捕捉ほそく効果こうかは,粒子りゅうししょうPAHsよりもガスしょうPAHsのほうがおおきい可能かのうせい示唆しさされた.
